Children’s Management Plan

 "My Asthma Plan"  is a tool to help young people monitor and manage their asthma. The plan contains four sections; personal and emergency contact details, a three zone asthma management plan, an education checklist and the 5 minute rule: what to do if you have an asthma attack. My Asthma Plan is easy to follow and is suitable for children under 12 years of age.

 
Children are encouraged to fill out My Asthma Plan under the supervision of a parent, guardian or healthcare professional. The plan helps monitor the changes in the child’s asthma symptoms and gives information on what to do when their asthma is well controlled, not well controlled or out of control.
 
By keeping track of your asthma symptoms and the medication you take, the plan will help give you and your parents a clear understanding of when to ask for your doctor or asthma nurse's advice and what to do in case of emergency. The education checklist section of My Asthma Plan should be filled out by your doctor or asthma nurse at each asthma check-up.
